fix: [ENG-2842] fix mcp loop error high cpu and harden shutdown (#664)

- crash handlers: one-shot guard + isolated try/catch + exit(1) (#660)
- bounded shutdown via runMcpCleanup (2s cap, used by all 3 paths)
- McpServer.close() after daemon disconnect so in-flight tools can
  surface a clean error before the stdio transport closes
