docs(readme): move dotenv setup from quick start to configuration section

- Removed .env/.env.local setup instructions from Quick start
- Added new "Configuration -> Environment variables via .env files" section
- Clarified that dotenv loading is optional, with precedence and opt-out env var

This removes unnecessary distraction from the Quick Start Flow.
