Fix receive cursor skipping unread messages in send (#145)

* Fix inline send delivery cursor advancement

* Preserve external send delivery and monotonic acknowledgments

* Preserve relay reply references in inline send output

* Share inline receiver check, note unread overflow, speed up send delivery tests

- send: take the capped batch via a small InlineBatch (shared cap constant,
  forward-only ack after output), and end truncated output with a
  '+N more unread' note in place of the --name-only preview
- cli_context: inline_receiver() replaces the duplicated codex/adhoc check;
  fix the comment claiming codex has no delivery hooks
- tests: queue messages via SQL (~210s -> ~20s), drop review_ prefixes

* Keep router delivery for sends that fail before persisting

send only owns inline delivery once its message is persisted; earlier
failures (bad args, validation) fall back to the router's codex/adhoc
delivery as before. Add a process-bound --from delivery test.

* Notice messages that arrive after send takes its batch

Restores the end-of-send unread check (previously --name only) for every
inline receiver, skipped when the remaining note already covers it.
