CI: upload failed real-tool test dirs; enable Claude debug log

A Windows real_claude approval-gate failure showed the approved Bash
command still running after 42s, but the preserved test dir (and any
Claude-side log) was lost with the runner. Keep failed dirs under a
known path, upload them as an artifact, and have Claude write its
debug log into that dir.
