Refresh tool-activity mappings; report OpenCode/Kilo tool calls

Mappings checked against current upstream sources:
- Gemini: delegate_to_agent -> invoke_agent
- Kimi: add AgentSwarm (detail from its description)
- OMP: add task; ast_edit left out (takes glob paths, not a file)
- Codex: shell tools all reach hooks as Bash
- OpenCode/Kilo: bash/shell, edit/write/apply_patch/patch, task/subagent

OpenCode and Kilo never reported tool calls, so their mappings were unused.
The plugin's tool.execute.before (OpenCode 1) and tool.hook("execute.before")
(OpenCode 2) now pass the tool and args to `opencode-status --tool
--input-json`, which derives the detail from the spec (filePath, path,
patchText). This replaces the file.edited report. tool:patch joins the
--file filter contexts.
