PTY: start delivery on time, and see Claude ready in every permission mode
Claude's only ready pattern was "? for shortcuts", which non-default permission modes replace with "<mode> on (<binding> to cycle)". Those launches fell back to the 5s delivery-start timeout, and on Unix that check only ran on the next 10s poll timeout once the screen went quiet, so hcom term, delivery and launch_blocked were unavailable for ~11s after every launch. - ready_pattern becomes ready_patterns (any one matches); Claude adds "to cycle)", and Codex's hard-coded "» " exception becomes its second pattern. - Cap the poll timeout at the time left before the delivery-start fallback.
