fix(team-core): reap inbox locks after Windows pid reuse

Parked member revive was blocking session_start on withLock because
detectStaleLock treated a recycled live pid as the still-living owner
and waited 15s, so switch_session never acked inside the 20s residency
budget. Identify this process with a per-process instance id so a
predecessor lock for the same pid is stale.
