fix: drop the todo tool aliases and plan prompt

OpenCode 2 has no todowrite or todoread tools, so the TodoWrite/TodoRead
aliases and the "ALWAYS write the plan with todowrite" note were dead code
that only applied to OpenCode 1.x.

Remove both.

Tested with the smoke case asserting neither reaches the query starter.
