fix(context): resolve legally indented Markdown headings (#584)

Resolve legally indented Markdown headings in context lookups. CommonMark allows up to three leading spaces before an ATX heading, but heading-address lookups only matched headings at column 0. Indented headings now resolve; lines inside fenced code blocks are still ignored, and existing lookups are unchanged.
