feat(ui): mention picker heading and per-person avatar; ui 0.43.2 (#1576)

MentionSource.heading draws one heading row above the @ mention list and MentionPerson.avatar draws an image or tinted initials before each label; both optional, both absent for Plannotator, and a picker without them renders neither.
