Preserve OpenCode agent switching on older hosts (#1570)

The coordinated model handoff made switchModel support a prerequisite
for switching agents. Older V2 hosts that expose only switchAgent
therefore stopped applying the approved agent selection.

Follow OpenCode's sequential semantics instead: keep a successful agent
switch when model switching is unavailable or fails, and warn about the
model separately. A failed agent switch still prevents any model
attempt.
