test: isolate replacement hook tokens (#4194)

## Summary & Motivation

Derive the hook token from the run id instead of passing one in, so a watchdog replacement run never collides with the original's token registration.

## Test Plan

Existing e2e coverage runs in CI; ran `route-bundle-isolation.test.ts` locally and inspected the two failing run timelines.
