.squad: Wave 1 orchestration log + decision ledger merge

- orchestration-log: Add 3 Wave 1 entries (Ash reconciler spike, Parker manifest translator, Lambert test specs)
- log: Add Wave 1 session entry (all agents SUCCESS)
- decisions.md: Merge 9 inbox entries into canonical ledger, dedup (accessories→tags mapping open question noted)
- decisions/inbox: Delete merged files

Wave 1 Summary:
- Ash: React reconciler with VNode capture, 9 tests passing
- Parker: Manifest translator CLI, Windows filtering verified
- Lambert: 64 test specifications across 4 files

All deliverables staged for design review + Wave 2 orchestration.
