feat(claude): support explicit prompt cache options

- Honor `prompt_cache_options.mode = "explicit"` to suppress automatic `cache_control` insertion while preserving client-defined cache breakpoints
- Strip `prompt_cache_options` from request payloads before forwarding upstream and during token counting
